A person who declines one service has not necessarily declined every possible form of help. Accurate reporting depends on naming the option and keeping the statement within its evidence.

Make the object specific
“Declined support” is much broader than “declined the proposed appointment.” Include the particular option when it explains the plan or request.
Original scheduling example
Notes: Cannot attend the Tuesday group session; asked about a Saturday individual session.
Accurate: “She declined the Tuesday group session and asked about an individual session on Saturday.”
Misleading: “She refused further training.”
Do not supply a motive
If no reason is given, do not add one. When a reason is reported, attribute it appropriately and include it only if useful to the reader.
Spot the extra claim
“He declined the evening appointment because he is unmotivated” adds an unsupported judgement if the notes record only the declined appointment. The accurate version should stay with the documented choice.
Keep alternatives distinct
Interest in another option does not establish a booking. Use wording such as “asked about” or “would like information on” when that is the actual status.
Before finishing your practice letter, underline every refusal or preference. Check what was declined, whether a reason was supplied and whether the next step is confirmed or only requested.
