Present simple passive: the default formal

“Is monitored”, “are reviewed”, “is prescribed” — the workhorse of formal handover for routines and truths. Use for standing arrangements (“She is reviewed monthly”), ward truths (“Observations are recorded four-hourly”), and protocol statements (“Wounds are assessed using…”).
