20 Explained Models: Study Selection, Fidelity and Self-Review

Three lenses: selection, fidelity and self-review
Selection: which notes became sentences, and which were left out — with reasons. Fidelity: every model sentence traces to the notes without invention; time, status, responsibility and certainty preserved. Self-review: the checking moves that verify purpose, content and language before submitting. Read each model three times, once per lens.
Study protocol: attempt first, always
Open the model book only after your own attempt — models are possible answers, not scripts to memorise and not officially graded samples. Other selections and arrangements can be effective where they fulfil the task accurately. Compare for decisions: where did your selection differ, and whose serves the reader better?
Counting and form discipline
Models run approximately 180–200 body words: whitespace-separated counting, hyphenated expressions as one word, salutation/subject/address/closing excluded, short thank-you sentences counted where in the body. Counting supports practice; it implies no automatic pass threshold.
Demonstrations (newly written)

Three-lens study (newly written)
Model gist (illustrative): wound-care discharge, 180 words. Selection lens: dressing regime + tolerance + review date kept; admission vitals dropped. Fidelity lens: “twice-weekly” preserved exactly; no invented healing claims. Self-review lens: purpose sentence checked, request dated, articles verified.
Fidelity repair (newly written)
Draft: “She recovered well.” (invents outcome.) Repaired: “She remains under review with gradual improvement noted.” (status preserved, certainty honest.)
One model, three lenses, fully worked (newly written)
Labelling: the illustration below is newly written. Model gist (illustrative): 172-word referral for a non-healing leg ulcer. Selection lens: ulcer chronicity, size trajectory, vascular findings and diabetes kept; decade-old appendicectomy and stable statin dropped — kept items all serve the vascular-surgery decision. Fidelity lens: “twice-weekly dressings for six weeks” preserved exactly; no healing claims beyond “no improvement noted”; responsibility clear (“dressings by community nursing”). Self-review lens: purpose in sentence one, request dated (“within two weeks given deterioration”), articles verified on re-read. Three lenses, one model, complete study in under an hour.
Full worked performance with commentary (newly written)
Labelling: the dialogue below is newly written (fictional scenario). Scenario gist: discharge counselling after asthma admission, reliever overuse. Performance: “Thank you for talking with me before discharge. [open with purpose] … How many reliever puffs did a bad day take last month? [unpack without judgement] … The key point: needing it more than twice weekly means control is slipping — prevention, not more reliever, is the fix. [chunked explanation] … Shall we agree preventer mornings and evenings with spacer, diary, GP review in two weeks? [negotiated plan] … Could you show me the technique once more, and tell me the red-flag signs back? [skills + teach-back close] … Seek urgent care for speech-limiting breathlessness. [safety]” Commentary: no judgement on overuse, explanation chunked, verification doubled (show + tell) — a model answer with self-review notes attached.
Practice bank: 8 tasks with answers
Task 1. Attempt-then-compare any model. Answer guidance: attempt first, three-lens comparison after.
