Professional writing becomes clearer when it describes what happened. A label such as “careless” may add judgement while hiding the actual event.
The task guidance in Clinical Precision: OET Writing for Medicine recommends objective, respectful language. This guide develops a practical editing method.
Look for labels without evidence
Ask what observable action the sentence is meant to convey. If the notes record a missed appointment, write about that appointment rather than assuming the person's motivation.
Original neutral example
Judgement: “The learner is irresponsible.”
Given fact: The learner missed Tuesday's session because the shift ended late.
Revision: “The learner missed Tuesday's session after the shift ended late.”
Preserve attribution where needed
If the reason was reported by the person, make that clear when it matters. Do not present an interpretation as independently confirmed.
Rewrite the label
Draft: “She refuses to organise her time.”
Notes: Reports difficulty planning around changing shifts.
Revision: “She reports difficulty planning practice around changing shifts.”
Check whether the detail serves the task
Neutral wording does not automatically make a detail relevant. Include it when it helps the recipient understand the problem or requested action.
Review one paragraph for adjectives about the person. Replace unsupported labels with precise actions or reports, then remove details the reader does not need. The result should be respectful and informative without becoming vague.
